Stolen Passport

Following steps are required to apply for new passport. For expedited process please call us

Step 1

• Fill the online DS-11 Application
• DS-64 Application Form - Statement regarding lost or stolen passport.
• Proof of Citizenship - One of the following must be provided
  • Previous U.S. Passport more than 15 years old.
  • Certified Birth Certificate issued by state, county or city
  • Certificate of Citizenship or FS-240, DS-1350
  • Certificate of Naturalization
• Proof of identity - A valid driver license (with recent photos on it)
• Evidence of Immediate Travel (If expedite process is required) - Following 2 copies are required:
  • Travel itinerary inidicating reserved or confirmed
  • Copy of round trip ticket indicating travel to a foreign country.
• Two recent passport 2 x 2 inch photographs taken with in six months(Photograph Qualtiy Requirements)
• Government Passport Fees - Following fees are applicable:
  • Passport Application Fee USD 135
  • Acceptance Agent Fees USD 25
• Authorizaion Letter for avisaservices.com (that allows us to submit and receive your passport / application)

Step 2

Acceptance Agent Authorization - An acceptance agent will verify your identity and witness your signature. Click here to find your nearest local agent. Please be sure you take all the documents mentioned in Step 1 to the local agent.
