- Fill
the online DS-11 Application
Note:When filling out application you will be asked your mailing address, you have to fill out our address which is P.O. Box-3626, New York, NY-10163 in care of A-Passport & Visa Services. The next question will be "Is this your permanent address ?" You have to say "No". Then you will be asked to enter your permanent address. So you can enter your home address. Please print the application and make an extra copy. Do not sign the application,this will be done at the post office in the presence of
the officer .

Proof of Citizenship - One of the following must be provided

Previous U.S. Passport more than 15 years old.

Certified Birth Certificate issued by state, county
or city

Certificate of Citizenship or FS-240, DS-1350

Certificate of Naturalization

Proof of identity - A valid driver license (with recent
photos on it)

Evidence of Immediate Travel (If expedite process is required)
- Following 2 copies are required:

Travel itinerary inidicating reserved or confirmed

Copy of round trip ticket indicating travel to a foreign
country.

Two recent passport 2 x 2 inch photographs taken with in
six months(Photograph
