Following
steps are required to apply for new passport. For expedited

Step
1
- Fill
the online DS-11 Application
- Proof
of Citizenship - One of the following must be provided

Previous U.S. Passport more than 15 years old.

Certified Birth Certificate issued by state, county
or city

Certificate of Citizenship or FS-240, DS-1350

Certificate of Naturalization
- Proof
of identity - A valid driver license (with recent photos
on it)
- Evidence
of Immediate Travel (If expedite process is required) -
Following 2 copies are required:

Travel itinerary inidicating reserved or confirmed

Copy of round trip ticket indicating travel to a foreign
country.
- Two
recent passport 2 x 2 inch photographs taken with in six
months(Photograph

- Government
Passport Fees - Following fees are applicable:

Passport Application Fee USD 135

Acceptance Agent Fees USD 25
- Authorizaion

and receive your passport / application)
